Researchers developed an innovative hydrogel treatment for periodontitis that uses iodide ions to enhance drug delivery deep into gum pockets. The system effectively eliminates Porphyromonas gingivalis bacteria that hide inside cells, while simultaneously reducing inflammation and preserving mesenchymal stem cells needed for tissue regeneration. Periodontitis affects millions worldwide and increases risks for tooth loss and systemic diseases like cardiovascular disease. Current treatments struggle to reach bacteria hiding deep in gum pockets and inside cells, particularly the keystone pathogen Porphyromonas gingivalis that drives disease progression.

Researchers at Sichuan University engineered a novel hydrogel system using gelatin, poly-L-lysine, and iodide ions. The chaotropic properties of iodide help the gel penetrate cell membranes and deliver antimicrobial peptides directly to intracellular bacteria. The injectable gel adheres strongly to gum tissue and releases therapeutic compounds in a controlled manner.

In rat models of periodontitis, the hydrogel significantly reduced alveolar bone loss compared to controls. High concentrations of iodide (5-10 mM) at the application site enhanced bacterial clearance, while lower diffused concentrations (0.5-1.0 mM) reduced inflammatory cytokines and promoted bone formation. Importantly, transgenic mouse studies showed the treatment preserved Gli1+ mesenchymal stem cells, which are crucial for tissue regeneration.

This multifunctional approach addresses three key challenges in periodontitis treatment: eliminating intracellular pathogens, controlling inflammation, and maintaining regenerative capacity. The technology could transform periodontal therapy by providing targeted delivery to hard-to-reach areas while supporting natural healing processes. However, human clinical trials are needed to confirm safety and efficacy.
