This 2025 narrative review from Case Western Reserve University synthesizes evidence on protein needs in adults 65 and older. The standard RDA of 0.8 g/kg/day is insufficient for this group due to anabolic resistance — a blunted muscle protein synthesis (MPS) response to amino acids that worsens with age. The authors recommend 1.0–1.2 g/kg/day for healthy older adults and 1.2–1.5 g/kg/day for those with chronic or acute illness, rising to 2.0 g/kg/day in severe cases. Evenly distributing 25–30 g of protein per meal and adding a 40 g pre-sleep protein feed are highlighted as practical strategies to maximize daily MPS and support immune health, muscle retention, and bone strength in aging populations.
